Drug combo better for common type of metastatic breast cancer, study suggests

ScienceDaily (Aug. 1, 2012) ? Postmenopausal women with the most common type of metastatic breast cancer now have a new treatment option that lengthens their lives, according a study led by UC Irvine oncologist Dr. Rita Mehta and conducted by the Southwest Oncology Group. The findings appear in the Aug. 2 issue of the New England Journal of Medicine.

A combination of the two anti-estrogen drugs anastrozole and fulvestrant extended the median survival time of women with Stage 4 hormone receptor-positive metastatic breast cancer by more than six months compared to those who underwent standard treatment with anastrozole alone.

Mehta said the results of the Phase 3 trial are particularly exciting because "these patients have not had a new treatment that gave them an overall survival benefit in more than a decade."

Both drugs are currently used to treat breast cancer, though not in combination. Anastrozole (also known as Arimidex) reduces the production of tumor-promoting estrogen, while fulvestrant (Faslodex) interferes with the receptors that allow estrogen to signal cells to grow and reproduce and also accelerates the degradation of these receptors.

The researchers think it's these modes of action together that make the combination so effective against hormone receptor-positive breast cancer, a subtype that accounts for more than half of all breast cancers.

"The next step would be to try the combination in even earlier stages of breast cancer to see whether long-term cures could be increased at those stages," said Mehta, an associate clinical professor of medicine with the Chao Family Comprehensive Cancer Center and an oncologist with UC Irvine's Breast Health Center.

Study results were first presented at the 2011 CTRC-AACR San Antonio Breast Cancer Symposium in December. The SWOG S0226 trial was conducted by the SWOG clinical trials network, previously known as the Southwest Oncology Group.

World's northernmost coral reef found

When you think of coral reefs, you probably picture scuba divers gliding through warm, crystal-clear waters. And for the most part, you'd be right: more than 90 percent of the world's coral reefs are located in the tropics.

Now researchers in Japan have found what is ? so far ? the world's northernmost coral reef. Located off the coast of Tsushima Island at 34 degrees north latitude, the newly discovered reef is far different from other coral reefs and is 217 miles (350 kilometers) north of most others in the region.

"Coral reefs have been believed to develop under warm-water settings ? at least 18 degrees Celsius (64 degrees Fahrenheit) in winter. This setting is 13 degrees Celsius in winter (55 degrees Fahrenheit), which is unbelievably low," Hiroya Yamano, a researcher at Japan's National Institute for Environmental Studies, told OurAmazingPlanet. "The species, and thus seascape, is completely different from normal reefs."

Yamano's team tracked down this reef after poring over old manuscripts and interviewing local residents. Their sleuthing paid off, and they eventually found the 4,300-year-old reef in one of Tsushima's murky inner bays.

"The water in this bay is basically turbid" ? cloudy with lots of suspended particles ? "and water temperatures are low, especially in winter," Yamano said.

Both of those qualities make life difficult for most corals. But this reef is composed of a genus of corals called Favia, a massive brown coral type. Most reefs are made up of corals from the genus Acropora, which can be a variety of colors and grow in branching or platelike polyps. Favia species tend to tolerate cold, turbid waters better than Acropora do, Yamano said.

So why did this reef start building itself in such an unfriendly environment? The team isn't sure, but Yamano thinks the Tsushima Warm Current, a stream of warm water flowing along the northwestern coast of Japan, probably helped transport coral larvae northward into the turbid inner bays of Iki and Tsushima islands. Yamano thinks there may be many more undiscovered reefs in similar settings throughout the region.

Changing species, changing climate?
Reefs like this one might help researchers measure ecosystem changes in warming oceans.

Although the Tsushima and Iki reefs both formed in very cold waters and predominantly house Favia coral species, Acropora corals have begun settling near the reefs over the last 20 years. Comparing coral species in older parts of the reef to newly arrived corals might help scientists determine how climate change and warming waters are affecting these reef ecosystems, Yamano said.

His team's findings were published online July 12 in the journal Geology.

?They think they?re pretty good,? the executive vice president said of his team of 200 senior leaders at a Fortune 500 corporation. ?They keep telling me that ?their people? need help in presenting dry information. But actually ALL of them need help. They get too far into the weeds. They do a data dump. They use too much jargon. They take too long to get to the point. Their charts are too busy. It?s often unclear what they?re asking us to do. Their summaries are either missing key information?or missing altogether.?

Although I listened as if hearing these indictments for the first time, the complaints are all too familiar. If I had only 5 minutes to coach presenters, here?s what I?d tell them:

1. Forget the warm-up drill as an opener. ?Good morning. My name is ?? does not set you apart in the line-up of presenters or the marketplace. Start with a high-impact opening that immediately engages listeners in your topic.

2. Make your information or facts tell a story. Don?t just dole out data. Turn your ideas into communication. Take a viewpoint, and shape your information persuasively to lead to a specific message. Connect with an audience to push them to action or a decision.

3. Punch key points?do not swallow them. Avoid rambling on with repetitious statements. Say it; then stop.

4. Strive for simplicity. The ability to make a complex subject understandable to the layperson is the mark of an effective communicator.

5. Never use a $100 story in a three-minute time slot to make a nickel point. Stories make your points and information memorable?but they must be shaped, edited, and delivered well. The longer the story, the better the point must be.

6. Add a touch of humor, but make it relevant to the topic and the situation. A humorous anecdote, illustration, or one-liner adds an element of class and distinction. Humor also reduces resistance and opens minds.

7. Make your presentation both a performance and a conversation. Your passion, energy, and topic make the presentation a performance. Your natural speaking style and relaxed but confident body language make it a conversation. Gestures, posture, movement, facial expression?these all either support or sabotage the impact of your content.

8. Master the monotone monster. Vary your volume, inflection, pacing, and intensity to engage listeners. Do not put people to sleep with the pitter-patter of a monotonous delivery.

9. Use silences to underscore your meaning. Pauses convey your meaning and give your audience breathing room between ideas. Without them, listeners find it difficult to distinguish between major and minor points.

10. End with a wallop, not a whimper. Never just fade away with a comment such as ?That?s all I have. Any questions?? The rule of primacy and recency says that people remember best what they hear first and last. That means your opening and your closing are the points of highest impact. Craft them carefully to create the best return on your presentation.
